We’ll hear from Malinda Ellwood of the Center for Health Law and Policy Innovation, Harvard Law School and Treatment Access Expansion Project.  She will provide a general overview of the patient centered medical home concept, with more specifics on the Medicaid Health Home.  Liz Brosnan of Christie’s Place in San Diego will discuss the pioneering partnerships Christie’s Place has built with patient-centered medical homes to improve services for people with HIV.
